In this dynamic role, you will play a pivotal part in the intake and assessment processes for new referrals, ensuring comprehensive care plans are developed and executed. This LPN position offers flexibility and autonomy while utilizing your nursing assessment, care planning and case management skills to provide optimal services to individuals in home settings. Civil and kind team collaboration and communication is essential. Case Management and consulting nursing does not require the delivery of skilled nursing procedures.
