In this dynamic role, you will play a pivotal part in the intake and assessment processes for new referrals, ensuring comprehensive care plans are developed and executed. This LPN position offers flexibility and autonomy while utilizing your nursing assessment, care planning and case management skills to provide optimal services to individuals in home settings.
