Nonotuck Resource Associates, Inc. is looking to add a Care Manager to our growing Adult Family Care program. Primary Duties will include performing all duties per AFC regulations, Comprehensive Multidisciplinary Intakes and Assessments, as well any required or related trainings. Any and all additional duties and roles will be required, if requested. The Care Manager is responsible to provide oversight and supports while working closely with those receiving services, their families, health care providers and service partners to develop and enhance truly personalized services and relationships.

This RN position offers flexibility and autonomy while utilizing your nursing assessment, care planning and case management skills to provide optimal services to individuals in home settings. Civil and kind team collaboration and communication is essential. Case Management and consulting nursing does not require the delivery of skilled nursing procedures. Requirements for the positions include valid Ma. RN license, computer abilities, travel/organizational planning, flexibility, excellent communication, training, referral, and planning skills.  Flexible coverage for other nearby offices as needed.

A clear and comprehensive understanding of DDS/ABI, AFC and other community/ home care delivery services preferred. The ability to adapt to changes, modify and redirect processes is required.
