Nonotuck Resource Associates, Inc. is looking to add a Care Manager to our growing Adult Family Care program. Primary Duties will include performing all duties per AFC regulations, Comprehensive Multidisciplinary Intakes and Assessments, as well any required or related trainings. Any and all additional duties and roles will be required, if requested. The Care Manager is responsible to provide oversight and supports while working closely with those receiving services, their families, health care providers and service partners to develop and enhance truly personalized services and relationships.

Requirements for the positions include valid Ma. RN license, computer abilities, travel/organizational planning, flexibility, excellent communication, training, referral, and planning skills. Flexible coverage for other nearby offices as needed.

The responsibilities of the Care Manager position include, but are not limited to direct case management, participation in care coordination via the treatment team and supervision of Shared Living and Adult Family Care (AFC) services. A clear and comprehensive understanding of DDS/ABI, AFC and other community/ home care delivery services preferred. The ability to adapt to changes, modify and redirect processes is required.
This position combines a hybrid working model with ability to attend in person visits to homes and to meetings.
We are searching for a dynamic and experienced candidate who will provide positive leadership in the implementation of Nonotuck values and key agency principles in the provision of high-quality personalized supports to the people served by the program. This position will oversee our Plymouth, MA area services.
Responsibilities include shared supervision of Shared Living/Adult Family Care homes and Care Managers, participation in care coordination via the treatment team, participation and oversight of the Shared Living referral process and case management responsibilities.
