Nonotuck Resource Associates, Inc. is looking to add a Care Manager to our growing Adult Family Care program. Primary Duties will include performing Comprehensive Multidisciplinary Intakes and Assessments, as well any required or related trainings. Any and all additional duties of the CM role will be required, if requested. The Care Manager is responsible to provide oversight and supports while working closely with those receiving services, their families, care providers and service partners to develop and enhance truly personalized services and relationships. Requirements: Bachelor’s degree from an accredited college and 2 years recent experience working with the elder population or those having a disability. Bilingual in Khmer. Computer skills, organizational planning, travel, as well as excellent communication and team building skills are necessary. A broad working knowledge of Adult Family Care services and DDS services is strongly preferred.

Full Time: Shared Living, Waiver Service Supports, Adult Foster Care Nonotuck Resource Associates, Inc. is seeking a RN to provide case management for our expanding services encompassing Lawrence, MA and surrounding areas. This position offers flexibility and autonomy while utilizing your nursing assessment, care planning and case management skills to provide services to individuals in home settings. Case Management nursing does not require the delivery of skilled nursing procedures. Requirements for the positions include: RN license, bilingual(Khmer), computer abilities, travel/organizational planning, flexibility, excellent communication, training, referral and planning skills.
