Shared Living Provider Job Description

Qualifications

  • AGE - The minimum age for the Provider is 18. There is no maximum age limit.
  • HEALTH - The Provider must be physically capable of providing adequate care and training to a person with disabilities in their home.
  • EMPLOYMENT - No restriction, but a suitable plan for the supervision of the person served while the Provider is at work must be developed in writing and approved by the Program Director.
  • TRANSPORTATION - Providers must own and be licensed to operate car or van or have 24-hour access to a car or van.
  • COMPOSITION OF THE HOUSEHOLD - No restrictions, but the number, ages and characteristics of other family members will be assessed on an individual basis to ensure that adequate time, energy and attention will be available to the person to be served. Appropriateness of the household environment will also be assessed on an individual basis.
  • Exceptions to these requirements may be considered on an individual basis. Any decision will be based on its effect on the comfort, privacy and well-being of the person to be served.

Duties and Responsibilities

The Provider will be responsible for upholding and adhering to the key principles of the agency and using these value statements as a basis for all decisions made, which affect the people being served. In addition to this, the following outline of responsibilities are expected:

  • The Provider will, to the best of his/her ability, provide a warm, supportive, growth promoting environment, which allows the resident to do as much as possible for him/herself (including taking risks which may be necessary to promote his/her independence), and supporting the residents' right and ability to make decisions for him/herself, while providing supervision and guidance as needed.
  • The Provider should be able to work constructively with Nonotuck Resource Associates staff and other professionals to develop and implement a plan of service for the resident which attempts to meet the following needs: medical, educational/vocational, social/leisure, emotional, residential.
  • The provider should be available to meet at least monthly in his/her home with the Program Director and Assistant Program Director, and may also be required to participate in quarterly or semi-annual individual service plan meetings with others involved in providing services to the resident.
  • The Provider will be expected to attend a minimum of 16 hours annually of in-service training on topics relevant to delivering quality services.
  • The Provider should be able to accept advice and consultation from staff and others providing support services to the resident. The Provider will also be expected to offer his/her expertise and advice, as indicated.
  • With assistance and support from Nonotuck staff, the Provider will develop and carry out a plan of in-home training activities to promote the continued growth and development of the resident. He/She may be asked to keep records of training activities and progress, and will modify the plan, with help from Nonotuck staff, as the resident's needs change.
  • The Provider will accept and promote the resident's relationship with his/her family as well as with other significant friends or acquaintances, in order to benefit the resident's emotional well-being and social development.
  • The Provider will maintain confidentiality regarding the resident and his/her biological family.
  • The Provider will keep records requested by Nonotuck such as a log of resident SSI and personal fund usage, receipts for use of resident funds, progress notes on resident development, behavioral checklists, etc. The Provider should be able to present these in meetings with appropriate Nonotuck staff and others involved in providing services to the client.
  • The Provider will schedule and assist the individual in completing all medical and psychological appointments as indicated. The Provider will ensure that recommendation directives and prescriptions are implemented as directed by the physician.
  • The Provider will perform other duties and responsibilities related to caring for the residents as assigned by appropriate Nonotuck staff.
